New Patient Information
New patients are received by advanced appointment only.
We want to dedicate a specific time for our new patients so they may be seen without having to feel rushed and without being interrupted. In order to make your visit go as smoothly as possible, the appropriate new patient forms must be filled out in advance and delivered to our office prior to your visit.
Please take Notice: Completed new patient forms, additional medical records, and other information may require delivery to the clinic 48-72 hours before your new patient visit. This will allow Dr. Russell and the staff to extensively review your case records prior to your new patient visit granting a more focused personalized history and examination.
